Program Description Community support and independent housing for people with mental health related disabilities in the Salisbury and Elizabeth area. Support is provided in the areas of accessing and maintaining independent housing, daily living skills, community linkage, personal support to maintain health and well-being. MCL (North) provides support from 8 to 21 hours per week over 7 days. We are committed to planning support mutually with clients and are inclusive and considerate of their families, children, carers and significant other. We work in partnership with Northern Mental Health Services, SA Housing Trust, Community Housing agencies, carers and consumers to provide collaborative and needs based support. Other Community Living programs operate in the Southern metro area of Adelaide, the South East Region (based in Mt Gambier) and Whyalla (focus on services to youth). Funded by Dept of Human Services.
Evaluation Information Evaluation is based upon 3 monthly reviews with clients and partnership agencies related to the the outcomes for clients of the individual goal plans. Also evaluation occurs in relation to crisis contacts, hospital admissions and number of bed days in comparision with the previous 12 months without the MCL support.
Evidence Based on the pschosocial support programs operating in Victoria and NSW with the increased capacity to support over 7 days and up to 21 hours per week. Holistic approach to support and based on a compensatory model of support which provides the flexibility to increase and/or decrease the hours of support depending upon the needs of the client and . Support continues when people are in hospital and participation is voluntary and client centred.
Any other information Currently the eligibility criteria for receiving support is that a person must: Be 18 to 65 years of age. Have a mental health related disability. Have a key worker from the Northern Adult Mental. Health Services who can provide a referral. Have a connection to the Northern Suburbs of Adelaide.
